Please send us by mail the following documents:

  • Medical prescription for long-term condition (A.L.D) dated, signed and bearing doctor's stamp
  • Mention the description of the products reimbursed, their references, the quantities and the prescription period (within the limit of 6 months)
  • Non-refundable products may be mentioned on the lower part of the prescription or in a list presented separately
  • The photocopy of the certificate of social security rights in force,
  • Payment of delivery charges, plus the total amount of products not fully reimbursed.
